Day Retreat Group for Moms: Participation Agreement

Facilitators: Jacqueline Richards-Shrestha and Lauren Fishbein

This Participation Agreement (“Agreement”) is entered into by and between the undersigned participant (“Participant”) and the facilitators, Jacqueline Richards-Shrestha and Lauren Fishbein (“Facilitators”), in consideration of the opportunity to participate in the Day Retreat Group for Moms (“Retreat”). By signing this Agreement, the Participant acknowledges and agrees to the following terms and conditions:

  1. Purpose and Nature of the Retreat:
    The Retreat is designed to offer mothers a space for personal rejuvenation through various activities and group experiences. The Participant acknowledges that while the Retreat may offer healing benefits, it is not a therapeutic group and does not substitute for professional therapy or medical treatment.

  2. Cancellation Policy:
    2.1. Full Refund: The Participant may cancel their registration up to 14 days (2 weeks) prior to the Retreat date and receive a full refund.
    2.2. Partial Refund: The Participant may cancel their registration within 7 to 13 days prior to the Retreat date and receive a 50% refund.
    2.3. No Refund: Cancellations made within 6 days of the Retreat date will not be eligible for a refund.
    2.4. Emergency or Illness: In the event of an emergency or illness preventing the Participant from attending the Retreat, a credit for future services may be issued. This credit is valid for one year from the original Retreat date and may be applied to future groups or events organized by the Facilitators.

  3. Alternative Weather Date Policy:
    In the event that the Retreat is rescheduled due to inclement weather, the Participant is expected to attend the alternative date specified by the Facilitators. Please note that there will be no refunds if the Participant is unable to attend the rescheduled date.

  4. Assumption of Risk and Release of Liability:
    4.1. Outdoor Activities: The Retreat includes both indoor and outdoor activities. The Participant acknowledges that outdoor activities involve exposure to natural elements and potential hazards.
    4.2. Assumption of Risk: By participating in the Retreat, the Participant voluntarily assumes all risks associated with both indoor and outdoor activities.
    4.3. Release of Liability: The Participant releases and holds harmless Jacqueline Richards-Shrestha, Lauren Fishbein, and any associated entities, from any and all claims, demands, or causes of action arising out of or related to any injury, loss, or damage sustained by the Participant as a result of participation in the Retreat.
    4.4. Food and Dietary Needs: Lunch is included in the Retreat. While the Facilitators aim to provide snacks and meals that meet various dietary needs, they are not liable for any harm that may result from the food provided by the lunch provider.

  5. Use of Herbs and Plant Medicines:
    5.1. Herbal Activities: The Retreat may involve the use of herbs and plant medicines. While generally considered safe, there is always a risk of adverse reactions, including allergies.
    5.2. Assumption of Risk: The Participant acknowledges and accepts the risk of adverse reactions to herbs and agrees not to hold the Facilitators liable for any such reactions.
    5.3. Optional Participation: Participation in herbal activities is not required. The Participant is welcome to skip this activity, or any other activity, if they choose.

  6. Behavioral Expectations:
    6.1. Prohibited Conduct: Threatening, harassing, or otherwise inappropriate behavior, before, during, or after the Retreat, is strictly prohibited. Participants engaging in such behavior will be asked to leave the Retreat immediately without a refund.
    6.2. Respect and Consent: Participants are expected to treat each other with respect and dignity. Consent must be obtained before initiating any form of physical or emotional connection.
    6.3. Confidentiality: All information shared within the group is to be kept confidential. Participants agree to honor the privacy and sacredness of others’ experiences and discussions.

  7. Medical Disclaimer:
    The Participant acknowledges that the Facilitators are not providing medical or psychological advice, diagnosis, or treatment. The Participant should consult with their healthcare provider regarding any medical or psychological concerns.

Acknowledgment and Agreement:
By signing below, the Participant acknowledges that they have read, understood, and agree to abide by the terms and conditions outlined in this Agreement. The Participant also acknowledges that they are voluntarily participating in the Retreat and assume all risks associated with their participation.
